New Student Forms

Information and instructions for setting up your Swarthmore email and completing New Student Forms are e-mailed to new students in mid-May. In order to access and complete the forms online, you must first set up your Swarthmore network and email account. Students who are admitted later will be e-mailed that information approximately every two weeks. In mid-July, we will email you with academic placement information.

Below is a list of the forms that we ask you to complete. Unless otherwise noted, the new student forms should be completed online by June 1.

Required forms to be completed online:

  • Academic Advising Questionnaire

  • Emergency Evacuation Assistance Request

  • Computer Access Survey

  • Disability Accommodations Request Form: for those students who wish to disclose a disability, particularly any student who wishes to determine eligibility for an academic, housing, dining, or other accommodation.   • Health history forms: By June 30, complete required forms in the Student Health Portal. Visit the Student Health and Wellness new student website for more details. You must complete required health forms by the deadline in order to be eligible for new student housing and access to roommate information.

  • Health insurance: Annually, all incoming and returning students are required to provide the Student Health Center with proof of health insurance coverage.  

    • Students are enrolled by default in the Student Health Insurance Plan (SHIP).  

    • The SHIP premium is billed in one installment on the fall semester E-Bill issued in July.

    • Before August 1, you may waive out of SHIP if you meet the requirements of the College and provide proof of health insurance.  To waive out of the SHIP, follow the steps described in the Student Health and Wellness insurance information website

Optional forms to be completed online:

  • Religious/Spiritual Life Resource Request: if you would like to be contacted by a religious advisor or might want help getting to religious services off-campus, please complete this brief form
  • Swarthmore Social Justice Institute: The Summer Social Justice Institute is best described as a retreat and intensive workshop experience to help first-year students enhance both their leadership skills and their individual/group identity issues within the context of exploring issues of gender, race, sexuality, socio-economic class, spirituality, power, and privilege through the framework of storytelling.
  • FLI-POP: The First-Generation, Low-Income Peer Orientation Program, FLI-POP, will connect incoming first-generation, low-income, and/or undocumented students to current FLI student mentors.
  • Food Allergy/Intolerance Record: any student with a food allergy or intolerance who might need dining accommodations should follow the instructions found on the Allergies & Intolerances web page. After submitting the form, please call to make an appointment with Swarthmore Dining to discuss your specific needs, and to create a plan. For certain accommodations to the standard meal plan, the College may require medical documentation and may also connect you with our Student Health Service and Student Disability Service so that we can best meet your needs.
  • For those students who might work on campus: please go to the Student Employment information page and follow the instructions. You must have your Swarthmore account set up in order to access mySwarthmore.

Documents to bring with you to campus:

If you plan to have a student job on campus at any point in your Swarthmore career, then you must bring the required documents for student employment. Please be sure to note the requirements for original documents. The list is found on the Student Employment section under Campus Services in this website.
