New Student Forms
Information and instructions for setting up your Swarthmore email and completing New Student Forms are e-mailed to new students in mid-May. In order to access and complete the forms online, you must first set up your Swarthmore network and email account. Students who are admitted later will be e-mailed that information approximately every two weeks. In mid-July, we will email you with academic placement information.
Please complete all relevant forms throughout the month of June. The majority of forms are due starting June 15th, so please be aware of deadlines listed on the checklist. These forms include:
Required forms to be completed online:
- Personal Information, which includes pronoun information. Pronouns are an important part of respecting a person’s identity. We invite you to tell us your pronouns.
- Emergency Contact Information
- Emergency Evacuation Assistance
- Crisis/Confidential Contact
- Parent Information
- Housing Questionnaire: contact housing@swarthmore.edu with questions.
- ID Picture: upload an ID photo for your OneCard. If you have trouble uploading a photo, contact onecard@swarthmore.edu.
- Health Certificate and Health Insurance: required of all students. If you have any questions or trouble submitting the health forms, contact health@swarthmore.edu.
- Computer Access Survey
- Disability Disclosure Form: Submit this form if you wish to disclose a disability and request academic, housing, dining, or other disability accommodations. Please note that disclosing a medical condition on your health or housing forms does not replace the interactive process with Student Disability Services (SDS). To be considered for accommodations, you must submit a request to SDS and provide supporting documentation. Documents to bring with you to campus:
If you plan to have a student job on campus at any point in your Swarthmore career, then you must bring the required documents for student employment. Please be sure to note the requirements for original documents. The list is found on the Student Employment section under Campus Services in this website.
