How do I apply for a Social Security Number?

Social Security Number Documents Checklist:

1. Find employment On-Campus
2. Complete forms in JobX (forms that require a SSN, enter 000000000 to submit and then update when you get your number)
3. Get the Employment Verification Letter (see below) signed and dated by your supervisor as well as by a member of the ISC Team

Directions For Sample Letter:

1. Save a copy
2. Fill in ALL blank spaces
3. Take to your supervisor to sign
4. Bring to the ISC for us to sign
5. Complete the Social Security Application Form - SS5
6. Go to the Social Security Administration Office with all the documents listed in the checklist below.

Please review all instructions and make sure to have all necessary documents before going to the office.
