1. Please complete this 2024 SLP Registration Form for EACH child you have in the CCC.
2. Please complete this CCC SLP Bus Choice Form. If you have multiple children in CCC, you can complete one waiver & list all children’s names.
3. Please complete this 2024-25 CCC Waiver. If you have multiple children in CCC, you can complete one waiver & list all children’s names.
4. Please complete this CCC Photo/Recording/Interview Release Form. If you have multiple children in CCC, you can complete one waiver & list all children’s names.
5. TC & JC I ONLY! - Please complete this Swim Lesson Waiver for your Training Chorus or Junior Choir I Members. If you have multiple children in TC & JCI, you can complete one waiver & list all children’s names.
6. JC II ONLY! - Please complete this Science For Kids Waiver for your Junior Choir II Members. If you have multiple children in JCII, you can complete one waiver & list all children’s names.
